In enacting the Health Insurance Portability and Accountability Act of 1996 (HIPAA), Congress mandated the establishment of standards for the privacy of individually identifiable health information. This information, otherwise known as Protected Health Information (PHI), includes demographic, medical and financial information in respect to the health of a specific individual, the provision of health care to such an individual or the payment for the provision of health care to such an individual. This information can be in either oral, written or electronic form.
We are required by law to take reasonable steps to ensure the privacy of any PHI transmitted or maintained by us. We are also required to inform you about our uses and disclosure of Protected Health Information (PHI), your privacy rights with respect to your PHI , your right to file a complaint with us and the Secretary of the U.S. Department of Health and Human Services and the person to contact for further information about our privacy practices.
The privacy of the personal information we collect from customers and potential customers is very important to the agents and agencies listed above.
You have received this notice in accordance with federal and state laws. This notice is to provide you with our understanding of types of non-public personal information about you that we may collect, how we use it and how we protect that information.
I. Information we collect:
We collect non-public personal information about you from the following sources:
1. Information we receive from you on applications for insurance or from other
insurance forms you complete.
2. Information we receive from the companies we represent which provide
insurance policies to you.
3. Information about your transactions with us, the companies we represent,
or (our affiliates).
4. Information from other sources, such as employers or government
agencies.
5. Information from visits to our website.
The type of information we collect is related to the insurance you request from us and may include your name, address, email address, social security number, driver’s license number, marital status, health information, and other information required to get insurance coverages for you.
II. Information we may disclose:
Information we may collect and disclose for the purpose of acquiring, enrolling in and facilitating health, life and other insurance coverage includes: name, date of birth, social security number, phone number, address, email, marital status, drivers license number, medical history, prescription list, provider information, Medicare number and effective dates, other coverage I.D. numbers, and any other information specifically required for facilitating your insurance needs.
III. Information we share with third parties.
We do not share any non-public personal information except as permitted by law and required to fulfill our fiduciary responsibilities in assisting you with helping you obtain financial help to pay insurance premiums (including helping you set up a marketplace account, verifying identity and completing marketplace applications), understanding insurance needs to aid in selecting coverage, verifying enrollments, assisting in obtaining enrollment verifications, assisting in understanding billing and assistance with claim payments.
IV. Our procedures to protect your private information.
We restrict access to non-public personal information to those employees, agents, representatives or parties who need to know the information in order to provide the insurance products requested by our customers.
We have policies and procedures that give direction to our employees, and to agents and representatives acting on our behalf, regarding how to protect and use non-public personal information. In addition, we maintain physical, electronic and procedural safeguards to protect non-public personal information.
V. Retention Period and Revocation of consent
Our agency must retain documentation that included PHI for a minimum of ten years. If you would like to revoke consent for the disclosure of information for the purposes listed in section III you may do so by submitting a written request to:
